Secondo la definizione dell’American Thoracic Society, i conflitti di interesse — conflicts of interest (COI) — si configurano come “una divergenza tra interessi privati e obblighi professionali, tale da fare sorgere in un osservatore indipendente il ragionevole dubbio che le azioni o decisioni professionali siano motivate da un vantaggio personale, come ad esempio un guadagno economico, un avanzamento di carriera, un incremento dell’attività professionale o il prestigio sociale”. Nel processo di sviluppo di linee guida per la pratica clinica, le preoccupazioni non riguardano solo l’identificazione e la disclosure dei COI finanziari diretti o indiretti perché risulta spesso insoddisfacente la gestione complessiva dei COI, nonostante la loro identificazione e disclosure. 
In risposta alle richieste dei suoi membri internazionali, utilizzando sintesi di evidenze scientifiche e policy di organizzazioni internazionali, il consiglio di amministrazione del Guidelines International Network (CdA G-I-N) ha sviluppato uno standard per la disclosure e la gestione dei COI nel processo di sviluppo delle linee guida.
Tutta la comunità scientifica impegnata nello sviluppo delle linee guida, grazie a una crescente consapevolezza sull’importanza di dichiarare e gestire anche i COI indiretti, condivide approcci simili dai quali si possono evincere 9 princìpi chiave per la gestione dei COI. Nel formulare tali princìpi, il CdA G-I-N riconosce che non è possibile evitare completamente i COI nella scelta dei membri del panel per determinate linee guida o specifici setting; pertanto, la quesione fondamentale è una gestione imparziale, giudiziosa e trasparente dei COI.
